Despite an F.D.A.-approved safety test of embryonic stem cells in spinal cord injury that the Geron Corporation began in January, many scientists believe that putting stem-cell-derived tissues into patients lies a long way off. Embryonic stem cells have their drawbacks. They cause tumors, and the adult cells derived from them may be rejected by the patients immune system. Furthermore, whatever disease process caused the patients tissue cells to die is likely to kill introduced cells as well. All these problems may be solvable, but so far none have been solved.

Restrictions on embryonic stem cell research originated with Congress, which, each year since in 1996, has forbidden the use of federal financing for any experiment in which a human embryo is destroyed. This includes the derivation of human stem cell lines from surplus fertility clinic embryos, first achieved by Dr. James Thomson of the University of Wisconsin in 1998.

President Clinton contemplated but never implemented a policy that would have allowed N.I.H.-financed researchers to study human embryonic stem cells derived by others. Research was able to begin only in August 2001, when President Bush, seeking a different way around the Congressional restriction, said researchers could use any lines established before that date.

Critics said the distinction between the Clinton and Bush policies lacked moral significance, given that each was intended to get around the Congressional ban, based on a religious and moral argument. The proposed Clinton policy amounted to: Stealing is wrong, but its O.K. to use stolen property if someone else stole it. The Bush policy was: Stealing is wrong, but its O.K. to use stolen property if it was stolen before Aug. 9, 2001.

Mr. Obama has put the proposed Clinton policy into effect, but Congressional restrictions remain. Researchers are still forbidden to use federal financing to derive new human embryonic stem cell lines. They will, however, be allowed to do research on new stem cell lines grown in a privately financed lab.

Stem cell research is the best known of several avenues of investigation into what is known as regenerative medicine. To regenerate the aging body with its own subtle repair systems, of which stem cells are one component, would be far more effective than the brute methods of drugs and surgery used today.

But scientists are still merely at the threshold of understanding how the bodys 200 different types of cell interact with one another. It seems likely to be years before biologists know all the settings that must be adjusted in a human cells chromosomes to make it become a well-behaved cone cell in the retina or a dopamine-making neuron of the type destroyed in Parkinsons.

Despite the new interest in reprogrammed stem cells, human embryonic stem cells are still worth studying, both to track the earliest moments in disease and to help assess the behavior of the reprogrammed cells.

Gene Therapy – Sickle Cell Anemia News

Gene therapy is an experimental technique that aims to treat genetic diseases by altering a disease-causing gene or introducing a healthy copy of a mutated gene to the body. The U.S. Food and Drug Administrationapprovedthe first gene therapy for an inherited disease a genetic form of blindness in December 2017.

Sickle cell anemia is caused by a mutation in the HBB gene which provides the instructions to make part of hemoglobin, the protein in red blood cells that carries oxygen.

Researchers are working on two different strategies to treat sickle cell anemia with gene therapy. Both of these strategies involve genetically altering the patients own hematopoietic stem cells. These are cells in the bone marrow that divide and specialize to produce different types of blood cells, including the red blood cells.

One strategy is to remove some of the patients hematopoietic stem cells, replace the mutated HBB gene in these cells with a healthy copy of the gene, and then transplant those cells back into the patient. The healthy copy of the gene is delivered to the cells using a modified, harmless virus. These genetically corrected cells will then hopefully repopulate the bone marrow and produce healthy, rather than sickled, red blood cells.

The other strategy is to genetically alter another gene in the patients hematopoietic stem cells so they boost production of fetal hemoglobin a form of hemoglobin produced by babies from about seven months before birth to about six months after birth. This type of hemoglobin represses sickling of cells in patients with sickle cell anemia, but most people only produce a tiny amount of it after infancy. Researchers aim to increase production of fetal hemoglobin in stem cells by using a highly specific enzyme to cut the cells DNA in the section containing one of the genes that suppress production of fetal hemoglobin. When the cell repairs its DNA, the gene no longer works and more fetal hemoglobin is produced.

Gene therapy offers an advantage over bone marrow transplant, in that complications associated with a bone marrow donation now the only cure for the disease such as finding the right match are not a concern.

Twelve clinical trials studying gene therapy to treat sickle cell anemia are now ongoing. Nine of the 12 are currently recruiting participants.

Four trials (NCT02186418, NCT03282656, NCT02247843, NCT02140554) are testing the efficacy and safety of gene therapy to replace the mutated HBB gene with a healthy HBB gene. These Phase 2 trials are recruiting both children and adults in the United States and Jamaica.

Three trials (NCT02193191, NCT02989701, NCT03226691) are investigating the use ofMozobil (plerixafor) in patients with sickle cell anemia to increase the production of stem cells to be used for gene therapy. This medication is already approved to treat certain types of cancer. All three are recruiting U.S. participants.

One trial (NCT00669305) is recruiting sickle cell anemia patients in Tennessee to donate bone marrow to be used in laboratory research to develop gene therapy techniques.

The final study(NCT00012545) is examining the best way to collect, process and store umbilical cord blood from babies with and without sickle cell anemia. Cord blood contains abundant stem cells that could be used in developing gene therapy for sickle cell anemia. This trial is open to pregnant women in Maryland both those who risk having an infant with sickle cell anemia, and those who do not.

One clinical trial (NCT02151526) conducted in France is still active but no longer recruiting participants. It is investigating the efficacy of gene therapy in seven patients. For the trial, a gene producing a therapeutic hemoglobin that functions similarly to fetal hemoglobin is introduced into the patients stem cells. A case studyfrom one of the seven was published in March 2017; it showed that the approach was safe and could be an effective treatment option for sickle cell anemia.

I was raised in Eastern Europe among a family of physicians. Remembering my childhood, as far back as when I was only ve years old, I can still vividly recall accompanying my mother as she visited her private patients. The relationship-centered care she provided fascinated me. I could see that my mothers patients beneted greatly just from exposure to her grace and kindness. The European model of medical care I was exposed to was holistic encompassing much more than just prescription writing. In fact my mothers advice to her patients frequently drew from naturopathic medical knowledge.

As I grew up, a few instances of the holistic approach to patient care I witnessed as a child were particularly memorable. Still living in Eastern Europe, I recall when my newborn rst niece developed some problems that were relieved solely through lifestyle and dietary modications. When my older daughter was an infant and I began supplementing breast feeding with regular food, she began to suffer from bouts of diarrhea. Based upon advice from my pediatrician grandmother, I made a few simple modications to my daughters diet and her issues with diarrhea were solved.

In 1988 I moved to the United States, and became a licensed physician in 1993. Two years later, in 1995,1 commenced my residency. Throughout my professional career, I have always adhered to a model of practicing medicine where I look to treat the whole person, employing a holistic approach rst and without harm. I believe in a personal and caring relationship with my patients and derive great satisfaction from providing this type of patient-doctor connection.

Ten years ago when my father became sick with cancer, I started an intense study of the spiritual aspect of human existence along with physical, environmental, mental, emotional and social experiences. Unfortunately, my fathers cancer took his life before I had the chance to offer him the benet of my learning. Even though I could not help my father, my unique course of studies directly benet my current patients. I am condent that my many and varied acquired experiences, tempered by my emotional maturation, and are helping my patients today.

A PLATEAUA plateau lasts 4-6 days and frequently occurs during the second half of a full (40 day) course, particularly in patients that have been doing well and whose overall average of nearly a pound per effective injection (injection 3 and on) has been maintained. Those who are losing more than the average, all have a plateau sooner or later. A plateau always corrects itself, but many patients who have become accustomed to a regular daily loss get unnecessarily worried. A plateau does not mean that you are no longer responding normally to treatment.

For such cases we consider it permissible to break up the plateau. This can be done with an apple day. An apple day begins at 11am and continues until just before 9pm. The patients are instructed to eat six large apples whenever they feel the desire, generally one every three to four hours. During an apple-day no other food or liquids except plain water are allowed and of water they may only drink just enough to quench an uncomfortable thirst if eating an apple still leaves them thirsty. The apple-day produces a gratifying loss of weight on the following day, chiefly due to the elimination of water and loosening of impacted stool. This water is not regained when the patients resume their normal 500-calorie diet at lunch, and on the following days they continue to lose weight satisfactorily.

If you stop losing weight dont assume you are having a plateau call office to discuss with staff.

CONSTIPATIONIf you are not losing a pound a day, the most common cause is constipation. Keep hydrated to avoid constipation. An excess of water keeps the feces soft, and that is very important in the obese, who commonly suffer from constipation and spastic colon.

While under treatment we rarely permit the use of any kind of laxative except bulk forming fiber which is not a typical laxative as it is not chemically altering your retention of water. It is normal to have a reduction in the amount of bowel movements as compared to before, but you SHOULD be having at least one bowel movement daily. If not, this could account for a lack of weight loss. Sugar free Metamucil 20 minutes before lunch and dinner or two back-to-back colonics can be helpful.

If you have a long history of constipation (not having at least one bowel movement per day) we suggest that you eliminate ALL MILK products (yogurt, ice cream, and especially cheese). To rebuild an atrophied colon it may take a year of a dairy free diet and a great increase in fiber. To hasten results try doing a course of colonics (ten sessions $600).

COSMETICSWhen no dietary error is found, we look at cosmetics. What Is Regenerative Medicine?

Regenerative medicine is the catchall term for a burgeoning field of medicine that focuses on harnessing the bodys innate ability to heal itself in the management and treatment oforthopaedic injuries. Two of the most common procedures that fall under the umbrella of regenerative medicine is stem cell therapy and platelet-rich plasma (PRP) injections. While many clinicians believe that these procedures hold great promise, they are still in the very early stages of study and regulatory approval.

The ultimate goal of regenerative medicine in orthopaedics is to use the innate healing ability of a patients own body to regenerate diseased or damaged tissues. While regenerative medicine as a whole has applications well beyond the muscles, bones and connective tissues of the musculoskeletal system, orthopaedic regenerative medicine is specifically focused on delivering a specific set of cells to the site of an injury. These cells, through their natural anti-inflammatory and healing properties, in turn, act on the damaged tissue to help drive recovery.

Platelet-rich plasma (PRP) is the end product of a patients own blood that has been extracted and refined to create a far higher concentration of platelets than would normally be present. Best known for facilitating the clotting of blood, platelets, which contain hundreds of proteins called growth factors that reduce pain and inflammation, are now being recognized for the crucial role they play in the healing process. This concentrated preparation is injected into the site of injury and the platelets are allowed to go to work promoting healing in the injured area. PRP injections are always created from a patient's own blood, so there are never any foreign substances being introduced to the body during the procedure; it is simply a patients own blood that has been processed, without additives, to contain high levels of platelets, our bodys naturally occurring healing cells.

Stem cell therapy is a developing orthopaedic treatment modality that aims to harness the power of the bodys own stem cells to promote healing and recovery. Adult stem cells, which are found throughout the body, have many regenerative and anti-inflammatory properties and possess the unique ability to differentiate or change themselves. This allows stem cells to grow into a wide variety of cell types, including muscle, nerve, cartilage and even bone. Many orthopaedic surgeons believe that stem cells may have the ability to treat common soft tissue injuries and conditions, such as torn cartilage, arthritis, and muscle tears.

-Arthritis: Arthritis literally translates to inflammation of the joint, and describes any disease process which leads to cartilage loss. Arthritis in the knee is very painful because the injury to the cartilage of the knee can eventually lead to bone-on-bone contact. This leads to increased stiffness, pain, swelling and decreased the range of motion of the knee.

-Meniscal Injuries: The meniscus is a pliable, rubbery disc that distributes body weight equally across the knee joint. This connective tissue can tear if the joint undergoes stress or sudden impact, preventing normal functioning of the knee and causing pain and inflammation.

-Ligament Injuries: The knee has four major ligaments: the anterior cruciate ligament (ACL), posterior cruciate ligament (PCL), medial cruciate ligament (MCL), and lateral cruciate ligament (LCL). Sprains or tears to these ligaments can occur gradually over time or through acute injury to the joint; in any case, these injuries cause pain and instability in the knee and may need to be treated surgically. However, regenerative medicine can be used alongside or in place of traditional surgical treatments in some cases.

-Rotator Cuff Tears: The rotator cuff, a group of four muscles that attach the humerus (upper arm bone) to the shoulder blade, is one of the most crucial components of the shoulder. It allows the muscles and tendons of the shoulder to lift and rotate the humerus and is essential to almost every basic and complex movement we require of the joint. The rotator cuff can be injured through acute injury or it can tear gradually over time; in any case, injury to the rotator cuff is very painful and severely limits the shoulders ability to move and function as intended.

-Arthritis of the Shoulder Joint: Arthritis describes any disease process which leads to cartilage loss. Cartilage serves an incredibly important role in joints of the body, as it cushions space and prevents bone-on-bone contact. Arthritis can be very painful because the wearing down of cartilage causes bones to grind against one another. This leads to increased stiffness, pain, swelling and decreased use of the shoulder.

-AC Joint Separation: The acromioclavicular (AC) joint is the point in the shoulder where the collarbone (clavicle) and the acromion of the shoulder blade are connected by ligaments. An AC joint separation occurs when these ligaments are strained or separated, typically from a sharp blow to the shoulder. Grade 1 separations generally involve a sprain of the AC ligament, while Grade 6 separations involve total tears of the ligaments and noticeable deformity in the shoulder.

-Labral Tears or Degeneration: The labrum is a ring of firm fibrous tissue that surrounds the shoulder socket (glenoid) and stabilizes the joint. When the labrum is torn or degenerates, it can cause pain, stiffness, a decrease in athletic ability, and a popping or clicking sensation with movement.

While the safety and efficacy of regenerative medicine procedures are still under review, it is believed that there are some major advantages to these procedures versus traditional treatments, particularly surgery. First, these procedures are far less invasive than traditional surgical approaches and are therefore done in the outpatient setting. There are no incisions required for these procedures and, as such, the risks of infection and complication are dramatically lower than surgery, and recovery is very minimal. Oftentimes the only immediate side-effect of the procedure is soreness at the site of injection. Second, stem cell therapy requires very little, if any, pain medication during recovery, so there is no risk of adverse side effects to opioid medication. Given that we are currently in the midst of an opioid epidemic on par with some of the worst drug scourges in our nations history, keeping patients off of narcotics during recovery is no small matter. Finally, stem cell therapy relies on the bodys own cells to drive recovery, so there are no foreign chemicals, additives or implants being injected into your body.
