Ashley Eckstein took to the stage during Star Wars Celebration for Sisters of the Force to celebrate the women of the Star Wars universe.

Almost as soon as the voice of Ahsoka Tano took to the stage for the Sisters of the Force panel, she told the mostly female audience that Star Wars is for everyone. The actress couldnt be more right. All one has to do is look at the online vitriol. This has resulted in Star Wars actors deleting their social media accounts. After all, it only takes is a few bad apples to ruin the fun for everyone.I think back to the Episode IX panel. When Kelly Marie Tran was introduced, she got a rousing ovation and applause. This is the audience reminding the actress that theres still a fan base with love for her and her character, Rose. Eckstein would later invite Athena Portillo, Catherine Taber, Amy Ratcliffe, Vanessa Marshall, and Suzie McGrath to join her on stage.

We all have our own stories as to what draws us to the long-running saga. I may be a transgender woman but Im just as much of a Star Wars fan as I was before coming out a few years ago. My gender identity doesnt change my fandom. I think back to the surreal experience that I had when my name was called among the wait-listed outlets last year at SXSW for The Director and The Jedi red carpet. I did a double take as I could not believe that I got a spot on the red carpet. To be within feet of Luke Skywalker himselfMark Hamilland maybe even talk to him in person! Unfortunately, he went into the theater after tying his shoe. As such, he bypassed the four outlets standing on the side. But to be within a few feet of one of your heroes is absolutely insane!

We all have our own stories. Mine dates back to my cousin showing us Star Wars: A New Hope in the early 1990s. My uncle would later take my brother and myself to see the special editions in 1997. I only turned 12 a few months earlier. Prior to watching each film, my uncle would give me the original VHS tapes. This is the only version of the original trilogy that I own. By which, I mean before George Lucas started to make any changes. I would later get the Special Editions on VHS, DVD, and then on Blu-ray when the Star Wars saga was packaged together in 2011. These films combined with Jurassic Park set me on the path to a hopeful filmmaking career.

At the same time as I discovered the Star Wars franchise, I was making another discovery of my own. Well, I would have had the education and awareness been around. It was around the the same time when I wrote my first piece of trans fiction. I turned it in for homework without as much of a thought to it. Being a writer since as long as I can remember, Ive even dabbled in writing a Star Wars parody or two. I never really did the whole fan fiction thing. But beside that, my go-to wall calendar each year is either The Beatles or Star Wars. There may be a Marvel movie mixed in with the bunch but its almost always a Star Wars calendar.

I would start buying the books during middle school as well. As each movie came out, I would by the novel tie-ins to see what insight they offered. This was before the days of audio commentaries in DVD so I wanted to learn just how it was that Qui-Gonn Jinn became a force ghost. His body never disappeared like Obi-Wan Kenobi and Master Yoda. Unfortunately, the books at the time didnt offer much but I digress.

Ive watched the Star Wars films who knows how many times. As such, I do my best not to give into fear, anger, or hate because it could lead to the Dark Side. This is one of those lasting mantras that I try and associate with my daily life as a result of these films. Its because of this that I have to pull myself away from social media. Mostly because of self-care.

Im a big Padme fan mostly because of Natalie Portman more so than Catherine Taber in Star Wars: The Clone Wars. I grew up at a time when none of us really knew what being transgender meant because it wasnt really covered in the press. Funny enough, Ashley was having members of the panel share something they never shared before. Im about to open up so please bare with me.

When people asked me who my celebrity crushes were, I always said Natalie Portman. One could say that this was true from a certain point of view. The thing is, I wanted to BE her but refused to really admit my gender issues at the time. When you grow up as a straight girl in the wrong body, things can be super confusing. Society tells you to act in a way that deep down, your subconscious knows you shouldnt. So no, I never got to wear the Princess Leia buns or dress as Padme on Purim. Purim is the Jewish holiday where people dress in costumes. Coming to terms with myself wouldnt happen until a few weeks before the release of The Force Awakens. It would not be an understatement to say that shit hit the fan.

Let me tell you, my first time watching The Force Awakens was not easy because the dysphoria was awful in December 2015. Like I knew I needed to have already started on hormone replacement therapy but the reality was that this is easier said than done. Watching the film prior to seeing The Last Jedi went much better because most of those dysphoric feelings were largely gone or invisible. They still get bad from time to time. However, its nothing like the dysphoric depression that nearly killed me. I will say how excited I was to see Daisy Ridleys Rey following in Carrie Fishers footsteps. One of the reasons why Star Wars works is because of these kick-ass heroines. Just as the Force was awkening in Rey, something else entirely different was awakening within me after years of repression.

A few weeks before Rogue One was released, I was really struggling with depression during the first week of November. I know exactly what Vanessa Marshall means when she spoke about her depression struggles. Even though I had just changed my name, I was homesick and my family had just put down our 16-year old border collie/husky mix before I had the chance to say goodbye. Moreover, when youre the rare St. Louis Cardinals fan living less than a mile from Wrigley Field, the week became super depressing because the Cubs went on to win the 2016 World Series. I was a real mess when I saw Doctor Strange but what started to turn things around was seeing the signage for Rogue One and knowing that were getting a live-action take on the Star Wars: A New Hope opening crawl. This gave me hope and something to look forward to.

Getting the opportunity to attend and cover my first Star Wars Celebration was amazing. While I didnt have the finances to shell out on the photos or autographs that I wanted, the one positive highlight of not winning the Episode IX lottery was winning a limited ticket for a photo with Anthony Daniels. While Luke Skywalker walked a few feet in front of me last year, meeting C-3PO himself during Celebration is an opportunity Ill never forget.

During Wizard World Chicago in August, I got to meet and chat with Greg Grunberg, who appeared in the sequel trilogy.

Finally, there was the moment that came during the Toronto International Film Festival when I got to formally meet writer-director Rian Johnson (Go see Knives Out!) at the world premiere of Jojo Rabbit:

PPI Use and Periodontal Disease: What’s the Link? – Monthly Prescribing Reference

The use of proton pump inhibitors (PPIs) in patients with generalized, chronic, moderate-to-severe periodontitis (stage III to IV; grade B to C) was associated with less severe periodontitis, according to study findings presented at the 2019 AIBD Annual Meeting in Orlando, Florida.

For this study, researchers aimed to assess whether treatment with a PPI could potentially decrease bone loss in patients with periodontitis. Data from patients who were referred to the postgraduate periodontics program at the University at Buffalo between 2010 and 2017 was used to identify those with generalized, chronic, moderate-to-severe periodontitis (N=518). The significance of systemic factors such as inflammatory bowel disease (IBD), smoking, diabetes, hypothyroidism, and other autoimmune disorders, in addition to medication history (ie, systemic steroids, hormone replacement therapy), were taken into account in the final analysis. The severity of periodontitis was measured by calculating the percentage of teeth that had pocket depths of 6mm.

We found that there was an inverse relationship between use of PPI and severity of [periodontitis], independent of the systemic conditions noted above, the authors reported. Findings from the study showed that 13.1% of patients on PPIs had pocket depths 6mm compared with 19.9% of those not receiving PPIs, resulting in a 34.2% decrease in severity (95% CI 2.1%, 11.6%; P =.006). Similar results were observed in the unadjusted patient population (15.7% on PPI vs 22.6% not on PPI; P=.004).

The data indicate that more conservative periodontal therapy might initially be considered for IBD patients with advanced periodontal disease for whom PPI are prescribed as a component of their IBD treatment, the authors concluded.

Breast Cancer and Hair Dye: Is There A Link Between Coloring Your Hair and Cancer? – Parade

A recent study published in the International Journal of Cancer examined the use of permanent hair dye and chemical straighteners among black and white women with a sister with breast cancer in the U.S.

They found a higher breast cancer risk associated with the use of permanent hair dye and straighteners, with the risk being higher in black women.

The study left many women asking one big question: Should I stop dyeing or straightening my hair?

That question may be on your mind, too, especially if you have a standing appointment at your favorite salon. But experts caution that more research would be useful before theyd encourage you to go gray (or curly).

I think its too early to make a big widespread treatment recommendation based on it, says Dr. Megan Kruse, a breast medical oncologist with the Cleveland Clinic.

Dr. Elizabeth Arena, a surgical oncologist specializing in breast cancer atSurgery Group of Los Angeles, has a similar perspective, saying shes not quite ready to tell her patients to stop dyeing their hair just yet. She points out that the study was an observational study, and shed like to see more research.

But I do think its really interesting, she says. I think its really great that someones looking into these things.

One thing to consider is that all the study participants already had a close relative with breast cancer, which automatically puts them in a higher risk category.

Many experts applaud the researchers of the new study for delving into the issue.

Its definitely going to be something that women are thinking about, says Dr. Kruse, who has already begun fielding questions from patients about the implications of the study.

Medical oncologist Dr. Abenaa Brewster credits the studys researchers for delving into an issue that affects many women and for specifically including black women in the study.

They asked a relevant question that women are concerned about, she says, calling it a culturally relevant question for women.

As the study notes, more than one-third of women over the age of 18 dye their hair, and those dyes contain an array of chemicals, as do many chemical straighteners, which often contain formaldehyde, which is a carcinogen.

The researchers wrote that, in their findings, permanent hair dye use was associated with a 45% higher breast cancer risk in black women, compared with a 7% higher risk in white women. They also raised questions about products used by many black women. They wrote, The strength of association observed for permanent dye use among black women is consistent with toxicological assessments that report higher concentrations of estrogens and endocrine-disrupting compounds in hair products marketed to black women.

People can control how they alter their hair color or consistency. If future studies conclude that using certain types of hair products do raise the risk of developing breast cancer, some doctors may add that to their list of modifiable risk factors.

But there are already other modifiable risk factors.

There are things that you can do to alter your risk, and this may be one, says Brewster, professor in the Department of Clinical Cancer Prevention at MD Anderson Cancer Center.

For example, you cant change your age or your family history of breast cancer, but you can make positive changes to your diet. You can exercise more frequently, lose weight and maintain a healthy body weight. You can swap cocktails for mocktails, as research shows that reducing the amount of alcohol that you consume can lower your risk for developing hormone-receptor-positive breast cancer.

The use of hormone replacement therapy, or HRT, is another possible risk factor that some women can address. Many post-menopausal take some form of HRT to relieve menopause symptoms, such as hot flashes, sweating and vaginal dryness.

HRT does come in different formspills, as well as skin patches, gels and vaginal creamsand with different combinations of hormones, notes Dr. Kruse. And the risk may depend on the type of HRT that a woman chooses, the age of the woman herself, and the duration of time that she takes HRT.

Dr. Arena says she encourages patients with concerns to talk to their gynecologists about their issues with menopause. Some women may choose to forego HRT or switch to a different type, depending on the symptoms theyre trying to manage and their other risk factors, such as a family history of breast cancer.

But at this point, with patients who are at average risk for developing breast cancer, Dr. Arena says she will probably tell patients, At this stage, you dont need to change your practice with what you do with your hair.

Dr. Kruse hopes that women will take advantage of the opportunity posed by the studys release to consider their own risk profiles. She also hopes theyll look into their family histories, if they havent already.

It will hopefully get some people asking their family members about it and talking about it, she says.

IT IS KNOWN colloquially as the change. The end of a womans natural child-bearing years is a moment of transformation that is welcome to some and miserable for others. But for too many, menopause is also a painful process that can damage their bones, heart and brain. As societies age, the question of how best to preserve womens health during menopause is becoming more urgent. In 1990 nearly half a billion women were 50 or older (the age when menopause typically begins). Today there are almost twice as many.

About 47m women around the world reach the age of menopause each year. In Western countries, where most research has been conducted, up to 80% will experience symptoms such as hot flushes, night sweats, depression, insomnia, anxiety and memory loss. Symptoms can last up to 12 years. Around a quarter of women going through menopause feel so wretched that their quality of life is dimmed, according to studies in rich countries. Almost half of British women experiencing it say that their work suffers as a result.

Twenty years ago, doctors would routinely have prescribed hormone-replacement therapy (HRT) to women entering menopause. But in 2002 the results of a huge randomised trial were published, showing that the treatment brought health risks, including a slightly raised chance of breast cancer after five years. Women and doctors were alarmed. Around the world they abandoned hormonal therapy in droves. Before the study, 22% of menopausal women in America took HRT. Six years later that figure had fallen below 5%. In Australia, around 15% of menopausal women with moderate or severe symptoms receive the treatment. Take-up of HRT is now low in most countries. Women are scared and doctors wary.

And yet the conclusions of the study in 2002 were rapidly debunked (see article). A re-examination of its results showed that women aged between 50 and 59 who took HRT were 31% less likely to die of any cause during their five to seven years of treatment with the hormones than those who did not. For a woman who has had her uterus removed or who starts menopause before the age of 45, HRT greatly reduces the risk of heart disease, a life-saving effect. It can also prevent osteoporosis, a disease in which bones become brittle. One study of post-menopausal American women over a ten-year period found that, of those who had had hysterectomies, between 18,000 and 91,000 died prematurely because they had shunned hormone therapy. HRT also lowers womens risk of uterine and colon cancers. Fears about the increased risks of breast cancer have been overplayed.

Hormonal therapies are typically off-patent and inexpensive. In Britain the annual price tag is only 125 ($165); in the United States generic pills are similarly affordable. And the benefits vastly outweigh the costs. Nothing else controls the symptoms of menopause so well, and a heightened risk of any one disease must be weighed against the lowered risks of contracting several others.

Hormonal therapies are not appropriate for all menopausal women. For some, the symptoms are insufficiently severe for it to be worthwhile. The treatment might not be suitable for those with liver disease, or a history of blood clots, or breast or ovarian cancer. But for serious symptoms, alternative treatments are worse than taking HRT. Herbal supplements, yoga and faddy dietsto which some turn in the absence of medical helpmay alleviate the unpleasantness of menopause but do not offer the long-term health benefits of HRT. Moreover, the symptoms can portend serious health problems in the future. Doctors could usefully prescribe HRT far more widely than they do today.

When Dr. Izzy Lowell moved from Boston to start her new job at Emory University Hospital in Atlanta in 2013, she quickly realized something was missing.

The family medicine clinic she worked with didnt seem to be serving transgender patients, which seemed unusual to Lowell.

Working with the LGBTQ community had always been something she had been interested in doing, even though her medical school training did not really cover how to treat people on hormone replacementtherapy (HRT).

She realized that to serve this population she would have to educate herself and learneverything she could about hormone replacement therapy. Soon she put in place a one-day gender clinic at Emory reserved for those patients.

Pretty soon patients started driving in from out of state, she said.

Even in one of the largest cities in the South, Lowell discovered, that medical care focused on this population and on transgender people in particular, was severely limited.

She watched patients crossing state lines every week to visit the clinic.

I thought, there has to be a better way to treat underserved transgender people from out of state, she said recently.

Fast forward five years and Lowell found a solution.

Her health organization QueerMed, which she founded in 2017, is an evolution of the one-day clinic she ran at Emory. In part, it operates as a pop-up clinic providing hormone replacement therapy for transgender and non-binary people. Lowell and her team of two nurse practitioners will go to towns in states across the South where she is licensed to do consults and start patients on HRT. Additionally, they offer HIV care and prevention and other primary care services. Follow-up visits are mainly done via teleconferencing.

Word of mouth has kept the doctor and her team busy with pop-up clinics scheduled once or twice a month in cities like Chattanooga, Tenn.,and Montgomery, Ala., where gender affirming care can be difficult if not impossible to find.

Even in Georgia, a state with the fourth highest population of people who identify as transgender, gender affirming health providers arent the norm.

Its a huge issue. I had a patient last week call 10 other offices in Atlanta to start hormone therapy. They all said, Im not going to do that, said Lowell. If someone has to call 11 different offices in Atlanta for care, imagine what its like in small towns?

While access to gender affirming health providers is a problem across the U.S., its a significant problem in the South, which is home to the largest population of people identifying as transgender in the U.S. New data released in early November by the Movement Advancement Project, showed that the southern states are home to about 567,000 transgender residents. Thats about 41 percent of all U.S. adults who identify as transgender.

Health care discrimination is unlawful under existing federal law and the Affordable Care Act, which took effect in 2014, also prohibits discrimination. Still, the transgender population is historically underserved and face significant barriers in the healthcare system including access to providers and medical staff who are culturally competent. Other transgender people face financial barriers and prejudice. These issues are often worsefor transgender people of color, in particular black transgender women who face higher incidentsof violence.

Its estimated that about 23 percent of transgender people avoid seeking medical care for fear of being discriminated against and about 33 percent say they cant afford healthcare, according to the 2016U.S. Trans Survey from the National Center for Transgender Equality.

The LGBTQ advocacy group, Campaign for Southern Equality, led a focus group recently to better understand the healthcare needs of transgender and non-binary people in the South. They spoke with 48 participants from North Carolina, South Carolina, Alabama, Mississippi, Arkansas, and Tennessee who reported barriers to finding and accessing affordable care. For instance, finding providers who are available and willing to offer care.

This includes the staff in the waiting room, says Ivy Hill, the community health director for the Campaign for Southern Equality.

I cant emphasize the importance of a knowledgeable front office staff, Hill said. Sitting in the waiting room and having the person out you as trans, in a rural small town where everyone knows you? Suddenly you are outed to everyone in that community. It cannot be overly emphasized how dangerous that can be.

Kayla Gore, a resident of south Memphis, transitioned from male to female when she was 23. Without insurance at the time, she started the process on her own without medical help buying hormones off the street. This resulted in her developing serious blood clots in her legs, a potentially fatal complication that can result from taking hormones without medical supervision.

When she landed a job that offered health benefits she was able to start HRT under the guidance of a doctor.

The process was challenging, Gore said, requiring a lot of extra testing because of her history with blood clots. She was also required to speak with a mental health counselor once a month to be assessed for gender dysphoria the medical term used to describe a person whose gender identity does not match up with the biological sex they were assigned at birth.

No one explained to me why I was seeing this person, Gore said describing the doctors tone and demeanor as condescending. She eventually switched to a younger doctor who had a better understanding of how to work with transgender patients, Gore explained.

We have thought about these decisions for weeks, months, a lifetime, Gore said. I always had gender dysphoria. Its hard to go to the doctor and feel questioned about something you have always known about yourself.

Gore, who is now 34 and working for the Transgender Law Center and as an organizer for the advocacy group Southerners on New Ground, says that finding a job with health benefits was a turning point in her story.

Without insurance she estimates that HRT in the Memphis area averages between $150 to $175 per treatment, not including the cost of lab tests and additional fees.

For some people, thats nothing. For many, its impossible, she said.

Even if the cost isnt prohibitive, Gore estimates there are only about five doctors in the Memphis-area that offer hormone treatment. The wait to see them is enormous, Gore said. This forces a lot of people to use the black market or go online to start it on their own.

Louisiana resident, Jules Morales, 26, said he had tremendous support from his friends and family which made it easier for him when he started the gender transition process from female to male three years ago.

A friend directed him to Dr. Brandy Panunti, an endocrinologist, who over the last three years, has led efforts at the Ochsner Health System in New Orleans to offer HRT and health services aimed at transgender people.

Morales was on HRT for a year before he decided to undergo gender reassignment surgery. Unable to find a doctor who could perform the surgery in Louisiana, Morales flew to California after a nearly 8-month waiting period. He remained there in recovery for nearly a month after the surgery.

Im lucky. I had tremendous support from my friends and family and it made all of this a lot easier, Morales said.

The team at Louisiana Trans Advocates receives about two to three emails a day from transgender people looking for help identifying food assistance or other resources in Louisiana.

About 90 percent are looking for a doctor, said Dylan Waguespack, the president of the board of directors for the organization.

Most are looking for providers able to offer hormone replace therapy so that they can get a documentation change. As a result, the group keeps a close track on what providers offer these services in Louisiana.

While cultural competency can be a barrier, with providers not necessarily knowing how to work and address transgender people, the bigger issue is access.

There arent enough providers offering transition related care, Waguespack said.

There are a handful of health providers in Louisiana that have made strides to improve care for transgender people. CrescentCare in New Orleans opened a gender clinic providing primary care and hormone replacement therapy in 2017. The Planned Parenthood clinic started offering these services in October.

Ochsner Health System, the largest hospital network in Louisiana, has made cultural changes to educate staff and health providers. This includes training employees on how to address the health needs of transgender people.

One of the most significant changes they made was to create an alert in their electronic medical record system that notifies providers when the person identifies with a gender they werent assigned with at birth. Using preferred names was an easy practice for staff to incorporate, Panunti said.

They also updated their medical coding to make sure there werent any errors and a patient could receive coverage for gender specific exams.

Since making these changes, Panunti said that several primary care physicians in the Ochsner system have reached out to start offering HRT. Transgender patients have traditionally gone to endocrinologists for HRT, but in recent years more internal medicine and family physicians have started providing this care in their practices.

When you talk about caring for the gender patient its not complex, Panunti said. But really being an advocate for this population and helping them not feel alone from the walk-in through the parking lot, to the waiting room and minimizing their vulnerability. (That) needs to be at the forefront.
