Spinal Cord Injury Improvement After Stem Cell Therapy in Mumbai India
She is a known case of Spinal Cord Injury since age of 1 year. After trauma she lost sensations of motor power below D6 level and on bowel bladder. MRI report showed 1) tiny focal traumatic contusion in cervical and upper dorsal spinal cord. 2) Grade 1 listhesis at C6-7 level. 3) Thin pre vertebral and anterior epidural hematoma. Initially she got good physiotherapy rehabilitation. She recovered in terms of bowel and bladder sensation, 50%. She has developed scoliosis (convexity towards right side) due to weakness of paravertebral muscles. Neurologically, she is hypertonic and hyperreflexic. She has ankle clonus. On examination: she has sensory loss below T6 level. She has grade 0 muscle power in bilateral lower extremities and near normal upper extremities strength. She is following time voiding for urination and she passes stool once in every 2-3 days. She has no pressure sores. On ASIA scale she scores "A". Functionally, she needs assistance in most ADL and wheelchair bound for mobility. On WeeFIM she scores 53. After Stem Cell Therapy 1. Exercises tolerance has increased. 2. Sitting posture is better. 3. She sits more erect now. Earlier, she used to stoop to one side. 4. Parents report that she responds to pain (being pinched on the side) now. Earlier would not be able to appreciate. He is a paraplegic has history of fall from height in 1993, leading to fracture D4 and paraplegia with complete bowel bladder loss. He was managed conservatively and has been undergoing rehabilitation since then. Neurologically, he is hypertonic and hyperflexic. On examination: he has sensory loss below T4. He has grade 0 muscle power in bilateral lower extremity and near normal upper extremity. He has no bowel and bladder control and is on condom catheter. He has bilateral knee flexion contracture =10. On ASIA impairment scale he scores #39;A #39;. On investigation: MRI: 6/7/2012: Wedge compression of the D4 vertebral body with a focal area of scarring within the spinal canal involving the arachnoid and the spinal cord at D3-4 level. A large cord syrinx extending cranially upto C4vetebral level with a smaller syrinx extending caudally up to D7 vertebral level. EMG / SSEP: It is suggestive of a severe lesion affecting the posterior column conduction from both lower limbs. There is evidence of predominantly Upper motor neurone lesion affecting the lower limbs. Functionally, he needs assistance in most ADL and wheel chair bound for mobility. He has history of fall in the swimming pool ,leading to fracture and compression of C4 over C5 and Quadriplegia with complete bowel bladder incontinence . He was operated for C5 corpectomy spiral stabilization. He is on regular rehabilitation since then. Neurologically, he is hypertonic hyperreflexic On examination, he has partial sensory recovery till D1, but complete loss below D1 .He has grade 0 muscle power in bilateral lower extremity wrist below, but grade 3++ muscle power in bilateral shoulders. He has no bowel bladder control is on condom catheter for same on ASIA impairment scale he scores #39;A #39; Functionally, he is dependent for all ADL will chair bound for mobility. On FIM he scores 73 After Stem Cell Therapy 1) Trunk balance has improved in sitting and standing. 2) Touch sensation has improved (10%). 3) Now while standing and exercising, can feel stress coming on joint (proprioception). 4) Spasticity, flexor spasms in lower limb increased. 5) Left thumb movement has started. 6) FIM -- no change. 7) Now spasticity grade 2 to 3. 8) Walking in parallel bars better.
