Integrative Medicine Program | MD Anderson Cancer Center

The Integrative Medicine Program of the University of Texas MD Anderson Cancer Center offers two (2) areas of research in which an intern can choose:natural productsandclinical trials.

Interns will receive supervision, mentoring, and feedback fromPeiying Yang, MS, Ph.D.Assistant Professor at the Integrative Medicine Program. Interns will assist the research team with current or forthcoming pre-clinical research or clinical trials on natural products as cancer therapy or preventive approach. The goal of the project is to provide the opportunity for the intern to learn how to conduct evidence-based research in the context of natural products, cancer therapy or cancer prevention. Interns will utilize specific methodologies in order to assess and summarize the information on the current status of anti-inflammatory or anti-angiogenic research using natural products or nutritional supplements.

Lorenzo Cohen, Ph.D., Professor, Director of the Integrative Medicine Program, encourages and provides training and guidance to our Interns by serving as the primary program mentor. Amy Spelman, PhD, Research Program Director, provides the Interns with supervision, mentoring, and feedback. Interns are given the opportunity to experience the development and/or implementation of mind-body and acupuncture clinical trials conducted within the Integrative Medicine Program and the Department of Palliative, Rehabilitation and Integrative Medicine. They will be exposed to and learn all aspects of running a clinical trial, including study participant recruitment and follow-up, data collection and entry, and basic data analysis.

Appointments are solely for the purpose of research, and no patient care activities are permitted.

Research Internsmayattend conferences and lectures, observe clinics, the OR, and rounds, in accordance with appropriate medical staff policy.

Research traineesmay notperform a primary medical evaluation or a history and physical examination; participate in decisions concerning management of a patient; render treatment or other patient services; order tests or service; or write in the medical record.

Integrative Medicine Program offers three (3) intern sessions per calendar year. Up to 12 interns are approved for each session. Interns work varied schedules, Monday through Friday and, usually work between 8:00 a.m. to 5:00 p.m. Generally, Interns are not compensated; however, some financial opportunities are available (seeR25E Grant Funding Information).

*If an education credit is required, it can take up to 120 days to process your application through the office of the University of Texas MD Anderson Cancer Center's Academic and VISA Administration (AVA).

Step 1 Submit Initial Application to the Integrative Medicine Education Committee

Interested applicants must complete theResearch Intern General Information Form(doc) for review by the Integrative Medicine Education Committee. The completed form along with a copy of your curriculum vitae or resume must be submitted to Anthony Sturm, Senior Administrative Assistant, via email atASturm1@mdanderson.org.All documentation must be received by the deadline for the desired session.

Step 2 Integrative Medicine Education Committee Review

All requests will be processed within a 30-day period following the application deadline by the Integrative Medicine Education Committee. A personal and/or telephone interview with a member of the Integrative Medicine Education Committee may be required. An email confirming or denying your intern request will be sent as soon as a determination is made.

Step 3 AVA Review if Accepted by the Integrative Medicine Education Committee

If a candidate is selected, the applicant will receive notification along with an electronic link to AVAs formal online application process called DISCOVER. Be prepared to provide numerous documents to AVA as part of the vetting process. The AVA can take up to 45 days to process the required paperwork or 120 days if an education credit is required.

About Stem Cell Therapy – Kansas Regenerative Medicine

In regenerative medicine today, adult stem cells hold the most promise for cell-based therapies. There are two main types of stem cells, adult and embryonic, which require unique means of collection and deployment.

Early research in the field was often associated with the controversial use of embryonic stem cells. Today, the cutting-edge of the science focuses on Mesenchymal stem cells found in an adults blood, bone marrow and fatty deposits. At KRMC we never use embryonic stem cells.

Adipose Stem Cells

Cell therapy around the world is shifting its focus from bone marrow based cells to adipose (fat) derived cells. These cells are easy to obtain and are generally robust. Adipose fat is an abundant and reliable source of stem cells. The best quality adipose cells are derived from the enzymatic digestion of liposuctioned fat which can be performed in a nearly painless, outpatient procedure.

The quality of stem cells deployed appear to be related directly to the clinical success and favorable outcomes of a procedure. Once adipose derived stem cells are deployed into an affected area of a patient body, the cells have the potential to:

Bone Marrow Derived Stem Cells

While Mesenchymal stem cells derived from bone marrow offer the same benefits as Adipose derived stem cells, there are two reasons KRMC does not currently collect stem cells from bone marrow.

A Comparison: Bone Marrow and Adipose

Adult Stem cells are obtained by different methods, each offering a different experience. The two most common methods are the older, bone marrow derived method and the increasingly popular Fat (Adipose) derived method.

Whether adult mesenchymal stem cells come from bone marrow or from fat does not appear to make a difference in clinical results. Some centers claim that bone marrow derived cells are superior to fat derived cells but there is no clinical evidence to substantiate this claim. While there are more studies on bone marrow cells, there is no evidence as to their clinical superiority.

It is important to understand that the word bone in bone marrow is sometimes used to implying that an orthopedic source might be better for treating orthopedic conditions such as cartilage regeneration. In fact, bone marrow is part of the reticulo-endothelial system and happens to be found in the center of bone. All of these types of cells are equally undifferentiated and have the potential to differentiate into mature functional tissues.

For many disease types such as cardiac pathology, adipose derived cells appear to be showing superiority to bone marrow derived cells. This may be related to the well documented qualitative and quantitative attrition in bone marrow stem cell counts related to age and chronic illness (chronic disease causes bone marrow suppression). Such changes in the number of cells over time and the quality of cells dependent on health have not been seen in fat derived stem cells. Fat derived cells are a natural choice for our investigatory work considering their easy and rapid availability in extremely high numbers.

Embryonic Stem Cells

At KRMC, we never use embryonic stem cells. Embryonic (ES) Stem cells are isolated from the inner cell mass of blastocysts of preimplantation-stage embryos. These cells require specific signals to differentiate to the desired cell type. If simply injected directly, they will differentiate into many different types of cells, resulting in a tumor derived from this abnormal pluripotent cell development (a teratoma). The directed differentiation of ES cells and avoidance of transplant rejection are just two of the hurdles that ES cell researchers still face. In addition, Embryonic stem cells are associated with ethical considerations and limitations.

Amniotic stem cells

KRMC does not use amniotic stem cells. Commercially available sources of stem cells, such as amniotic products, are not collected from ones own body. These cells are gamma radiated by manufacturers and are, therefore, not live cells.

Researchers tested a kind of bone marrow stem cell to discover if it could help treat sepsis. They found that these stem cells could triple the odds of a person surviving the condition.

Sepsis is a life-threatening response to an infection. It is triggered when an established infection somewhere in your body (skin, lungs, urinary tract, etc.) causes an extreme immune response by your body. The aftermath sepsis leaves in its wake is shocking. In just a short amount of time, it can cause tissue damage, organ failure and death.

Fever, low blood pressure, increased heart rate, and increased breathing rate along with lab tests that check for signs of infection and organ damage are current methods used by doctors for diagnosing sepsis. However, since the symptoms of sepsis (like difficulty breathing and fever) mirror symptoms for other conditions, it can be challenging to effectively diagnose sepsis in its early stages.

While the first modern view of sepsis dates back to the early 19th century, little progress has been made in treating it.

Current protocols are to give antibiotics, maintain blood flow to organs, and treat the source of the infection. Patients receive oxygen and intravenous (IV) fluids to maintain blood flow and oxygen to organs. Sometimes surgery is required to remove tissue damaged by the infection.

Antibiotics are the go-to method of treatment and can offer some help, but researchers have been trying to find a more effective treatment option for centuries.

Now, stem cells are offering a new angle on fighting the fight against sepsis and infections.

It is common knowledge in the stem-cell science field that mesenchymal stem cells support immune system function and regeneration of damaged tissues. Scientists tested a theory that bone marrow stem cell could treat sepsis and discovered that stem cells could actually triple the odds of a person surviving a bout with sepsis.

Researchers conducted an experiment on a group of mice with sepsis. The level of sepsis the mice had in their bodies caused infection and organ damage. At the height of infection, the scientists administered injections of bone marrow stem cells to some of the mice, and not to others so they could compare the outcomes. Five days later, half the mice that had received stem cell treatment were still alive. Only fifteen percent of the mice that didnt receive the stem cell treatment were still alive.

It was also found that the mice that received the stem cell injections had better overall health, less inflammation throughout the body, and less bacteria. Their ability to fight infection was stronger than before the injections.

One of the most encouraging results of the experiment was how fast the stem cell worked on the sepsis state. Because of how quickly sepsis can attack tissues and organs, it is important to find a treatment option that allows for quick results.

The Centers for Disease Control report that 1.5 million Americans contract sepsis each year with 250,000 of that number dying. One in three of patients that die in the hospital have sepsis, says the CDC. A major cause of mortality in intensive care units is sepsis. By using stem cells to treat sepsis, these hundreds of thousands of lives that fall victim to sepsis each year could be saved, along with billions of dollars spent on immediate and on-going care for those that survive, but who survive with long-term damage.

The stem-cell treatment advantage is that it not only buoys the health of a specific organ or part of the body, but the mesenchymal stem cells also assist along the entire path the infection traversed, reducing the risk of long-term tissue and organ damage as well.

Diabetes is a life-long disease that affects the way your body handles glucose, a kind of sugar, in your blood.

Most people with the condition have type 2. There are about 27 million people in the U.S. with it. Another 86 million have prediabetes: Their blood glucose is not normal, but not high enough to be diabetes yet.

Your pancreas makes a hormone called insulin. It's what lets your cells turn glucose from the food you eat into energy. People with type 2 diabetes make insulin, but their cells don't use it as well as they should. Doctors call this insulin resistance.

At first, the pancreas makes more insulin to try to get glucose into the cells. But eventually it can't keep up, and the sugar builds up in your blood instead.

Usually a combination of things cause type 2 diabetes, including:

Genes. Scientists have found different bits of DNA that affect how your body makes insulin.

Extra weight. Being overweight or obese can cause insulin resistance, especially if you carry your extra pounds around the middle. Now type 2 diabetes affects kids and teens as well as adults, mainly because of childhood obesity.

Metabolic syndrome. People with insulin resistance often have a group of conditions including high blood glucose, extra fat around the waist, high blood pressure, and high cholesterol and triglycerides.

Too much glucose from your liver. When your blood sugar is low, your liver makes and sends out glucose. After you eat, your blood sugar goes up, and usually the liver will slow down and store its glucose for later. But some people's livers don't. They keep cranking out sugar.

Bad communication between cells. Sometimes cells send the wrong signals or don't pick up messages correctly. When these problems affect how your cells make and use insulin or glucose, a chain reaction can lead to diabetes.

Broken beta cells. If the cells that make the insulin send out the wrong amount of insulin at the wrong time, your blood sugar gets thrown off. High blood glucose can damage these cells, too.

While certain things make getting diabetes more likely, they won't give you the disease. But the more that apply to you, the higher your chances of getting it are.

Some things you can't control.

Some things are related to your health and medical history. Your doctor may be able to help.

Other risk factors have to do with your daily habits and lifestyle. These are the ones you can really do something about.

Because you can't change what happened in the past, focus on what you can do now and going forward. Take medications and follow your doctor's suggestions to be healthy. Simple changes at home can make a big difference, too.

Lose weight. Dropping just 7% to 10% of your weight can cut your risk of type 2 diabetes in half.

Get active. Moving muscles use insulin. Thirty minutes of brisk walking a day will cut your risk by almost a third.

Eat right. Avoid highly processed carbs, sugary drinks, and trans and saturated fats. Limit red and processed meats.

Quit smoking. Work with your doctor to avoid gaining weight, so you don't create one problem by solving another.

The symptoms of type 2 diabetes can be so mild you don't notice them. In fact, about 8 million people who have it don't know it.

Your doctor can test your blood for signs of diabetes. Usually doctors will test you on two different days to confirm the diagnosis. But if your blood glucose is very high or you have a lot of symptoms, one test may be all you need.

A1C: It's like an average of your blood glucose over the past 2 or 3 months.

Fasting plasma glucose: This measures your blood sugar on an empty stomach. You won't be able to eat or drink anything except water for 8 hours before the test.

Oral glucose tolerance test (OGTT): This checks your blood glucose before and 2 hours after you drink a sweet drink to see how your body handles the sugar.

Stem Cell Transplantation at Virginia Oncology Associates

Virginia Oncology Associates autologous stem cell transplant program is only one of three in the state of Virginia and 135 nationally. Sanctioned through the Foundation for the Accreditation of Cellular Therapy (FACT), the program adheres to the most stringent clinical standards. FACT accreditation is the gold standard insurance companies often demand. It is the benchmark for quality care.

A stem cell transplant, sometimes called a bone marrow transplant, is a procedure that restores blood-forming stem cells in the bone marrow. This is necessary when a patient has had their bone marrow damaged by cancer or by cancer treatment. Blood-forming stem cells are important because they grow into different types of blood cells. The main types of blood cells are white blood cells, red blood cells, and platelets. You need all three types of blood cells to be healthy.

Stem cell transplants are used most commonly for patients with blood cancers such as leukemia, multiple myeloma, and some lymphomas. In cancers such as these, high doses of chemo (sometimes in combination with radiation), are administered with the intent of killing the stem cells. This stops the process of making new blood cells and the cells required for your immune system. Once the treatment is complete, the patient will receive a transplant of stem cells from a donor. The cells are administered through the blood and then move to the marrow where they can begin their work of regenerating the necessary blood and immune cells.

Over time, the stem cells that have settled in the bone marrow begin to grow and make blood cells that are healthy. This process is called engraftment.

The blood-forming stem cells that are used in transplants can come from the bone marrow, bloodstream, or umbilical cord.

There are two main types of transplants, which are named based of who gives the stem cells:

If you have an identical twin, you may receive a less common type of transplant, called a syngeneic transplant. In this transplant, the stem cells are taken from your twin as long as you both have identical genetic makeup and tissue type.

The goal of an autologous stem cell transplant is to restore the bodys ability to make normal blood cells after receiving high-dose chemotherapy or radiation treatment. In this procedure, your own stem cells are harvested (removed) from your bone marrow or your blood before you get treatment that would destroy them. Once your stem cells are removed, they are frozen (cryopreservation). After you receive your chemo treatment, the stems cells are thawed and given back to you.

One big advantage of this type of transplant is that youre getting your own cells back. This means you arent at risk for graft versus host disease -- a medical complication where new stem cells (called the graft) attack your body. However, there can still be graft failure, which means the cells dont make blood cells as they should because they havent reached the bone marrow. Additionally, this type of transplant doesnt offer the benefits of graft versus tumor (GVL) effect, which is when healthy donor cells attack cancer cells.

There are also some adverse side effects to autologous stem cell transplantation, which are mostly a result of the high-intensity conditioning therapy. These effects may include:

Autologous stem cell transplants are mainly used to treat certain leukemias, Hodgkins disease, lymphomas, and multiple myeloma. In some cases, it is used for testicular cancer, neuroblastoma, and certain childhood cancers.

VOA uses autologous stem cell transplants.

People have different sets of proteins, called human leukocyte-associated (HLA) antigens, on the surface of their cells. The set of proteins, called the HLA type, is identified by a special blood test.

When a donor will be the source of stem cells for a cancer patient, the oncologist will aim to match the patients own stem cells as closely as possible. This can help minimize side effects.

In most cases, the success of allogeneic transplantation depends in part on how well the HLA antigens of the donors stem cells match those of the recipients stem cells. The higher the number of matching HLA antigens, the lesser the chance the patient will reject the stem cells.

Instead of working against cancer directly, stem cell transplants help you recover your bodys own ability to produce stem cells after chemotherapy and/or radiation treatment.

With that said, stem cell transplants may work against cancer directly in patients with multiple myeloma and some types of leukemia. For example, a stem cell transplant where stem cells come from another person can also help treat certain types of cancer in a way other than just replacing stem cells. Cells that are donated can often find and kill cancer cells better than the immune cells of the person who had the cancer ever could. This is called the graft-versus-tumor (GvT) or graft-versus-leukemia (GVL) effect, which simply means that certain kinds of transplants actually help fight the cancer cells, rather than just providing normal blood cells.

Although some patients could benefit from stem cell transplants, they are typically only used when all other first-line treatments have failed.

Even though stem cell transplants have been successful for many people, it is important to weigh the risks and benefits before making a decision. Your cancer care team should talk with you about the risks and potential benefits. They may also discuss other methods of treatment available, including clinical trials. Various factors, including your age, stage of the cancer, donor type, and overall health are all part of the decision-making process.

As with all other types of medical treatment, the final decision is yours regarding whether or not to move forward with a stem cell transplant.

Through stem cell therapy research, cancer stem cells (CSCs) are leading to an encouraging outlook on the future treatment of patients who have been diagnosed with germ cell tumors as well.

Germ cell tumors are growths that form from reproductive cells. They arise in the ovaries (in girls) and the testes (in boys). They can also sometimes occur in other areas of the body, such as the lower back (common in infancy), the abdomen, the chest, and within the brain.

Germ cell tumors may be cancerous or noncancerous. In most cases, cancerous germ cell tumors occur as testicular cancer or ovarian cancer.

Breast Cancer Stem Cells Stopping Them In Their Tracks

A growing area of cancer research, breast cancer stem cells were first identified in 2003 by scientists at the University of Michigan (UM) in the United States. With this article I will do my best to explain breast cancer stem cells, and offer some guidance about natural therapies that help to create healthy stem cells.

Healthy Stem Cells vs Breast Cancer Stem Cells

Think of healthy stem cells as mother cells that have the potential to become any type of cell in the body. One of the chief characteristics of stem cells is that they have the ability to self-renew or multiply while maintaining their potential to develop into other types of cells like blood cells or heart cells or bone cells. When the DNA of healthy stem cells becomes impaired or damaged, thats when they can become cancer stem cells.

Conventional cancer theory states that any cell in the body may undergo changes in their DNA sequences and become a rogue cancer cell. Researchers at the Ludwig Center at Stanford, however, postulate that our normal stem cells are the only cells that reproduce themselves and are therefore around long enough to accumulate all the necessary changes to produce cancer. 1

According to UM researchers, breast cancer stem cells (hereinafter BCSCs) are identified by a cell surface glycoprotein marker named CD44, which is responsible for cell to cell communication, cell adhesion and migration. Also on BCSCs are very low or no levels of a surface marker named CD24, another glycoprotein on the cell surface of most B lymphocytes, a white blood cell and part of the immune system. UM researchers discovered that just a few BCSCs were required for the growth and spread of breast cancer and that unless the BCSCs could be destroyed, tumors were likely to recur and spread to other parts of the body (called metastasis). 2

Although BCSCs comprise only a small population within a tumor, they are resistant to conventional chemotherapy and radiation therapy because BCSCs do not renew themselves through normal cell division as other cancer cells do. In fact, these therapies are known to stimulate BCSCs and make them grow faster. 3, 4

Let me repeat that. Chemotherapy and radiation do not kill BCSCs. Any cancer therapy that ignores BCSCs in favor of killing non-tumor causing daughter cells in order to shrink a tumor will not result in destroying the root of the cancer.

My friend Elyn Jacobs, a cancer strategist in New York City, advises: Dont be misled into thinking that tumor reduction guarantees that you are making progress with respect to overcoming the disease, as you may not be. Shrinking a tumor by killing the less damaging cancer cells (the non-stem cells) with chemo and/or radiation may NOT improve long term survival and may increase the likelihood that these cancer stem cells will metastasize, allowing them to regenerate tumors elsewhere in the body. So although you may get an 80% reduction in tumor bulk, you are not targeting the most dangerous of the cells. 5

The BRCA1 link

For those who have a mutation in the BRCA1 gene, this part may be of interest. As we know, the BRCA1 (or breast cancer 1) gene is a tumor suppressor gene, mutations in which are strongly linked to breast and ovarian cancers.

UM Cancer Center scientists have discovered that the BRCA1 gene regulates self-renewal of BCSCs. When the gene is mutated, BCSCs multiply abnormally.

So the task becomes clear we must take in the nutrients that help our stem cells and genes be as healthy as they can be. By doing this, we not only combat any BCSCs currently circulating in the body, we are also actively creating healthy cells and genes that have no damaged DNA.

Natural Therapies To Combat Breast Cancer Stem Cells

Glyconutrients.The exterior of most healthy cells are covered densely with things that appear to be tiny little trees, called glycoforms or glycans. These glycoforms prevent viruses, bacteria and other enemies from entering the cell by blocking the entrance to the cells surface. The glycoforms of healthy cells are also responsible for assisting in cell-to-cell communication, indeed they play an intimate role in nearly every single human physiological process. Glycoforms can also hold onto an enemy cell for safekeeping until an immune cell comes to destroy it (kind of like a holding cell at the police department!). The objective is to create healthy glycans/glycoforms so that our cells are able to communicate effectively, and this helps in a myriad of ways, including the formation of healthy stem cells and immune cells. In order to create healthy glycans/glycoforms we need to be consuming at least 8 simple sugars, known as monosaccharides, from our diet and the problem is we are only consuming two glucose and galactose. The other 6 sugars are missing because of things like fertilizers, pesticides, GMOs, harvesting fruit and vegetables too early before they are completely ripe thus most of the all-important simple sugars are either missing or extremely depleted. We absolutely must supplement with glyconutrients in order to build healthy cells that do the jobs they were meant to do.

Foods/Herbs that Help. Ginseng a chemical component called ginsenoside F2 derived from ginseng has been tested for its efficacy against BCSCs. Research found that ginsenoside F2 suppressed BCSC proliferation (rapid growth) and increased apoptosis (planned cell death). 4, 9 Raspberries (both red and black), and more specifically ellagic acid and ellagitannins, help our immune system find BCSCs by removing the protein that hides them from our immune system. While it may be tempting to take ellagic acid via a supplement, you must eat the berries because they contain the all-important ellagitannin complex within them. Raspberries also have many anti-mutagenic and anti-carcinogenic properties and are full of vitamins, minerals, antioxidants, they help reduce inflammation and encourage apoptosis. 10 Fermented soy products including soy sauce, miso, natto and tempeh have research indicating they help to reduce cancer symptoms, reduce chemotherapy side effects, decrease chemotherapy resistance, increase survival rates and improve cancer stem cell differentiation, suggesting reduced aggressiveness of cancer. 11

This by no means an exhaustive list, but the items listed above are definitely among the best things that are known to help create healthy stem cells rather than BCSCs.

This amniotic product was found to have no living cells by the Interventional Orthopedic Foundation (IOF). The SCIA continues to administer dead amniotic tissue under the guise of performing legitimate stem cell therapy with living and active stem cells. According to their own claims, the SCIA is providing living stem cell treatment to patients, using products that have been found to contain no living stem cells.
