A handful of people with type 1 diabetes have been able to survive without insulin shots for more than two-and-a-half years, on average, after having their own blood stem cells removed and reimplanted through intravenous injection, U.S. and Brazilian researchers reported Tuesday.

Patients had their own blood stem cells removed and reimplanted through intravenous injection.

Overall, the technique has been tried in 23 people, mostly boys and young men, who were treated within six weeks of a new diagnosis of type 1 diabetes. People who have type 1 diabetes, an autoimmune disease that destroys the insulin-producing cells in the pancreas, typically need insulin shots to survive. (People with type 1 make up only about 10 percent of all diabetics; most people have type 2, which can be controlled with diet, exercise, oral drugs, or insulin shots.)

After the transplant, 20 of the 23 patients became insulin-free for at least a few months or even years. Twelve of them stayed free of insulin for an average of 31 months and eight patients had periods ranging from six to 47 months in which they were free from insulin. One patient was free of daily insulin for more than four years before needing insulin shots, according to the report in the Journal of the American Medical Association. The stem cell transplants were performed at a single center in Brazil.

The new findings are "very encouraging," says Dr. Richard K. Burt, associate professor of medicine at Northwestern University's Feinberg School of Medicine, who presented the findings at a Washington press briefing. Still, he said, more study is needed before it's known whether the treatment truly alters the course of the disease. "I never use the word 'cure,'" says Burt, who is also chief of the Division of Immunotherapy and Autoimmune Diseases at Northwestern Memorial Hospital in Chicago, Illinois. "We just have to wait and see."

Two years ago, Burt and colleagues showed that 14 of 15 patients (who were also included in the new study) treated this way were able to go without insulin injections for one to 35 months, but it wasn't clear if this was due to closer medical follow-up and lifestyle changes. In the new study, the researchers measured a protein that suggested the patients did indeed have insulin-producing beta cells, which are typically destroyed by the autoimmune disease.

However, Dr. Mohamed El-Shahawy, a transplant nephrologist and a member of the islet cell transplantation team at City of Hope in Duarte, California, cited a number of concerns around the study and risks to patients.

"This study basically subjects patients to an unproven high-risk procedure," he said. "Could it be a breakthrough? I don't see it."

Chester was what veterinarians would call a lame dog. He had OCD in both elbows diagnosed at 8 months. Unfortunately, this disease is progressive. Improvement is expected with a treatment of medical and surgical management, but NOT normality because varying degrees of arthritic change already exist and will progress.

Chester was treated with one or more anti-inflammatory drugs once a day, plus something to protect his tummy from the medicine, as well as having the arthroscopic surgery in both elbows before he was a year old. Months after the surgery he was slow to get up and would often cry if we left him alone in a room. We knew he was still in pain.

At about 2 years old he had the stem cell treatment and he was back to his pre-diagnoses self!

Because Labs can act like puppies for a long time, he is now a big 100 lb baby and is full of energy . With the stem cell treatment Chester can again happily run and swim. He gets a walk to the park and to chase his ball daily. In the winter he goes with me to the beach and loves to body surf, something we used to have to moderate so he wouldn't get too sore. Since Vet-Stem's treatment Chester has been off all drugs too! It's truly amazing.

Although Chester is 4 years old now, it's fun to see our "puppy" running around and acting like the goofy Lab that he is.

Ryan Benton, a 28 year-old Duchennes muscular dystrophy patient from Wichita, Kansas, received his first umbilical cord tissue-derived mesenchymal stem cell treatment yesterday following US FDA approval of his doctors application for a single patient, investigational new drug (IND) for compassionate use.

Duchenne muscular dystrophy (DMD) is a rapidly progressive form of muscular dystrophy that occurs primarily in boys. It is caused by an alteration (mutation) in a gene, called the DMD gene, which causes the muscles to stop producing the protein dystrophin. Individuals who have DMD experience progressive loss of muscle function and weakness, which begins in the lower limbs and leads to progressively worsening disability. Death usually occurs by age 25, typically from lung disorders. There is no known cure for DMD.

This trial, officially entitled Allogeneic transplantation of human umbilical cord mesenchymal stem cells (UC-MSC) for a single male patient with Duchenne Muscular Dystrophy (DMD) marks the first time the FDA has approved an investigational allogeneic stem cell treatment for Duchennes in the United States.

Ryan received his first intramuscular stem cell injections from allergy and immunology specialist, Van Strickland, M.D at Asthma and Allergy Specialists in Wichita, Kansas. He will receive 3 more treatments this week on consecutive days. Dr. Strickland will administer similar courses to Ryan every 6 months for a total of 3 years.

This is not the first time Ryan has undergone umbilical cord mesenchymal stem cell therapy. Since 2009, Ryan has been traveling to the Stem Cell Institute in Panama for similar treatments. Encouraging results from these treatments prompted Dr. Strickland to seek out a way to treat Ryan in the United States.

The stem cell technology being utilized in this trial was developed by renowned stem cell scientist Neil H. Riordan, PhD. Dr. Riordan is the founder and president of the Stem Cell Institute in Panama City, Panama and Medistem Panama. Stem cell research in relation to anti aging medicine is truly revolutionary. Hemoglobinopathies, such as sickle cell disease and thalassemia major, are genetic diseases associated with significant morbidity and premature death. Allogeneic bone marrow transplantation (BMT) is the only potential cure for severe hemoglobinopathies. Typical regimens have used high doses of chemotherapy or chemo-radiotherapy to ablate recipient hematopoiesis and to prevent graft rejection. The widespread use of this treatment has been limited by toxicity, risk of end-organ damage, and donor availability. This study will use a nonmyeloablative regimen of fludarabine and busulfan to attempt to generate consistent engraftment with donor hematopoietic stem cells in patients with severe hemoglobinopathy.

G-CSF mobilization of the donor's peripheral blood white blood cells will precede donor apheresis. A nonmyeloablative conditioning regimen of fludarabine and busulfan will be administered to patients prior to allogeneic peripheral blood stem cell infusions. FK506 and prednisone will be administered for graft versus host disease (GVHD) prophylaxis. Patients will be evaluated for engraftment, donor: host hematopoietic chimerism, toxicity, and hemoglobinopathy.
