About Stem Cell Treatment

Early stem cell research has traditionally been associated with the controversial use of embryonic stem cells. The new focus is on non-embryonic, which is called adult stem cells found in a persons own blood, bone marrow and fat.

Dr. Oleszek is currently treating patients by a fat removal procedure that generally lasts approximately twenty minutes. Specially designed equipment is used to harvest the fat cells and less than 100cc of fat is required. Post operative discomfort is minimal and there is minimal restriction on activity.

The stem cells are then harvested under sterile conditions using a special closed system technology so that the cells never come into contact with the environment throughout the entire process from removal to deployment. Sterile technique and antibiotics are also used to prevent infection.

Depending on the type of treatment required, stem cells can be injected through veins, arteries, into spinal fluid, subcutaneously, or directly into joints or organs.All of these are considered minimally invasive methods of introducing the stem cells, but Dr. Oleszek has if focused on treating joints (if youre a candidate for knee replacement surgery, then you could possibly be a candidate for stem cell treatment). Stem cells injected intravenously are known to seek out and find areas of tissue damage and migrate to that location thus potentially providing regenerative healing.

Novel approach for estrogen-receptor-positive breast cancer reported

Loyola researchers and collaborators have reported promising results from a novel therapeutic approach for women with estrogen-receptor-positive breast cancer.

The new approach, a new drug class called gamma secretase inhibitors (GSI), specifically inhibits Notch and shuts down critical genes and cancer cells responsible for tumor growth.

Kathy Albain, MD, FACP, who led the study, will present findings Dec. 11 during the 2014 San Antonio Breast Cancer Symposium.

Existing cancer drugs are effective in killing mature breast cancer cells. But a handful of immature breast cancer stem cells are resistant to such drugs. They survive and are responsible for tumor growth and progression. Resistance to standard therapy is a major cause of death in women with estrogen-receptor-positive breast cancer. Approximately 75 percent of breast cancers are estrogen-receptor positive.

"New treatments are desperately needed for women with estrogen-receptor-positive breast cancer who develop resistance to standard therapies," said Dr. Albain, a professor in the Department of Medicine, Division of Hematology/Oncology at Loyola University Chicago Stritch School of Medicine. "Our research suggests a potential role this new experimental drug class may have in optimizing existing endocrine therapies, such a tamoxifen and aromatase inhibitors, and in overcoming resistance to cancer drugs."

The Notch protein promotes tumor growth and survival. The protein is present on the surface of cancer stem cells. The protein latches on to other cells, and the resulting "molecular handshake" activates various genes in the stem cells that drive tumor growth, spread and survival. Activating these genes, in effect, makes the stem cells resistant to common cancer drugs. A pilot study conducted at Loyola found that the GSI appears to block this process by turning off key genes.

The purpose of the study was to identify critical genes involved in the process. The study included 20 patients with early-stage, estrogen-receptor-positive breast cancer. Prior to surgery, the patients received one of two commonly used drugs, tamoxifen or letrozole, for 14 days to block the estrogen stimulation of breast cancer cells. They underwent a biopsy on day 14. They then received the GSI, MK-0752, plus continued one of two standard drugs, tamoxifen or letrozole. Patients underwent their definitive breast cancer surgery on day 25 and part of this tumor was provided as well for this research.

Stem cells are used to treat disease or injury. These cells take on the characteristics of tissue they are introduced to, and therefore begin to repair the tissue by replacing damaged cells. Recent breakthroughs include repair to skin, muscle and cartilage, with nerve tissue next.

Up until recently the media has largely focused on the more controversial embryonic stem cells. These are stem cells derived from embryos. The potential uses of these cells to cure and treat diseases remain vast, but the controversial source of the cells poses ethical questions that have delayed medical progress.

During the past decade, researchers have discovered that stem cells can also be extracted from your own fat tissue. These adipose-derived adult stem cells have the same potential as embryonic stem cells, except for the ability to differentiate into sperm or ova. Unlike embryonic stem cells, stem cells extracted from your own fat are abundant, easily available, and pose no ethical or political controversy.

COSMETIC ENHANCEMENTS vs. DISEASE THERAPIES

The FDA does not allow doctors to promote the use of stem cell therapies to treat diseases unless you are part of a clinical trial. In this capacity they are considered to be drug therapy and subject to very strict regulation. Therefore, many patients currently travel outside of the U.S. to get these therapies.

On the other hand, the use of your own (autologous) stem cells to restore a more youthful appearance is available now. These therapies do not fall under the FDAs drug therapy guidelines, because they do not involve drugs, merely your own fat being re-injected in the same operative session as the liposuction procedure to remove the fat. Also, the stem cells have been only minimally manipulated, and their use to enhance fat transfer does not alter the original biologic mission of the stem cell.

However, if your autologous stem cells are re-injected in a separate or second operative session, it is unclear whether the FDA considers that application to be a drug therapy, even though it is used for the same cosmetic purpose.

STEM CELL ENHANCED FAT INJECTIONS

Surgeons have been extracting fat cells since the early 1990s for the purpose of re-injecting them into other tissue for over a decade. The process has not always worked perfectly, however, and surgeons have learned that successful fat grafting is highly dependent on the techniques used. As it turns out, some methods for extracting, processing, and re-injection fat cells results in high concentrations of adult stem cells in the fat. This has produced not only long lasting results, but also repairs in injured tissues

To perform stem cell enhanced fat grafting, fat is first extracted by liposuction. The fat is then processed to isolated, concentrate and activate the stem cells. Techniques include centrifugation and chemically processing the extracted fat. Long lasting results are only achieved with proper fat processing techniques. The fat must also be re-injected in very small quantities and distributed through all tissue layers to facilitate reestablishment of blood flow to the fat grafts.

What are Embryonic Stem Cells and Adult Stem Cells?

The identification of stem cells as "embryonic" and "adult" refer to their origin.

Embryonic Stem Cells are found within the human embryo at the blastocyst stage (7-14 days after fertilization). They are pluripotent stem cells. These embryonic stem cells are amazing because they will multiply and differentiate to make up every cell of the human body, from those in the blood to those in the bone. It is truly a testament to the beauty of life that these simple looking cells can create the entire structure of the human body.

Adult stem cells can be found in many places within the human body. It can be found in bone marrow, umbilical cord blood, placenta, brain, skeletal muscle, hair follicales, fat, and many others places. Up until recently, adult stem cells were always referred to as multipotent stem cells, that is, they could only differentiate into the tissue cells of their origin tissue. Because of this, proponents of embryonic stem cell research look down upon adult stem cells because of their "limited" characteristics. However, as recent studies have confirmed, scientists have begun to unlocked the pluripotency of adult stem cells.

These adult stem cells are isolated from the patient, multiplied in the lab, directed towards the cell needed, and inserted at the place where tissue needs to be repaired or replaced. Because these stem cells are original to the host, there is no rejection by the immune system. Because they are retrieved from a certain type of tissue, there is no threat of multiplication in teratomas.

As we have indicated, embryonic stem cells, on their face value, are truly a beautiful and amazing part of human development since they are the foundational cells for every cell in the human body. And because of the pluripotency of embryonic stem cells and the corresponding supposed hope for medicinal use, researchs have sought to isolate these hESC. However, because theses stem cells are a necessary part of the embryos development, isolating the hESC necesitates the destruction of the embryo, which turns what was beautiful into a disgrace.

In human embryonic stem cell research (hESCR), the beauty of the human blastocyst is destroyed for the sake of using these hESC to treat other diseases. The stem cells of the human blastocyst are removed, leaving the blastocyst dead and unable to continue its maturation process. The image at the right indicates this process.

These cells are then placed in a pietri dish and are encouraged to multiply undifferentiated until a stem cell line is created. Then, these lines are either immediately frozen for later experimentation, or they are experimented on to see how they differentiate into specific cells, at what rate the differentiation is done, and how the differentiation can be controlled.

While many scientists see hESCR as the holy grail of medicine, it is alarming to note that neither any treatments or cures have materalized through the use of hESCR. In fact, they omit that their is no timeframe as to when treatments or cures will materialize.

Many proponents of hESCR cite the lack of funding by President Bush as the reason no successes have come forth.

However, there are significant problems with hESCR:

Because of these two problems, researchers need to research more to diagnose these problems. To research more, scientists need more human embryonic stem cells, which, of course, means more destrution of embryos to obtain the stem cells.

Lets break down how scientists isolate hESC

This newly created zygotes cells divide and develops on the same path as the egg fertilized by the sperm, meaning in 7-14 days it is called a blastocyst. At this point, this cloned embryo can either be implanted in a surrogate mothers womb and carried to term, or it can be stripped of its embryonic stem cells and left to die. (mcc video- 5:49-7:31)

Whether youve risen to the ALS Associations Ice Bucket Challenge with enthusiasm, or its oversaturation on social media has turned you into a wet blanket, local marketing experts said the craze has undeniably become a game changer for fundraising campaigns.

Sure, theres been some backlash. Some say its become more about vanity than charity. In the past two days, meanwhile, the Newark archdiocese, and Catholic dioceses in Ohio and Louisiana, have expressed concerns over the ALS Associations funding of research involving embryonic stem cells.

Nonetheless, not since breast cancers invention of the pink ribbon has a charity found such an explosively successful way to raise money and public awareness, according to industry observers. Other charities will have to do more than just come up with the next viral gimmick. Theyll have to invent something as innovative and groundbreaking, whatever it might be. Either that, or capitalize on the recent backlash against the craze.

But what have Facebook, Twitter and donating money ever been but a means for people to make themselves look good, marketing professionals ask. As long as it raises awareness and money, everybody wins.

How to explain why things go viral? In many ways, said industry observers, it isnt anything new. That bucket challenges surfaced on a grass-roots level and spread virally traces back to a business phenomenon as old as the hills: word of mouth, said Professor Ashwani Monga, chairman of the Marketing Department at Rutgers Business School in Newark.

"As we know, word of mouth matters most in any successful product or method. So in that sense, it has definitely worked," he said.

But why has the act of pouring cold water over our heads, for all the world to see, been such a digital draw? Again, observers said, the concept is steeped in timeless traditions born long before the advent of YouTube. Remember that concession at the school fundraising carnival, where you throw a ball and try to dunk your school principal in that big pool of cold water? Sound a little familiar?

"Theres no such thing as an original idea," said Anthony Torre, co-founder and chief marketing officer with Spitball Advertising in Red Bank. "Its a matter of transforming the idea in such a way that it becomes interesting again."

Both the carnival dunking booth and the ice bucket challenge share the same crucial selling point, he said: "Both are public forums in which we can make people look silly."

Except that now, rather than its being confined to the school principal, just about anyone who is anyone is willing to look silly if it means being seen on the Internet. And yes, while donating to a worthy cause. Locally, participants have run the gamut everyone from Bergen County Executive Kathleen Donovan, to Governor Christie, to various county freeholders, police officers and fire captains, some mayors, the Ridgewood High School boys soccer team, Audi Meadowlands General Manager Chris DeMarsico and 31 of his employees. And thats just to name a few.
