New UCI-led study finds that your genetic sex determines the way your muscle talks to other tissues in your body – Newswise

Newswise Irvine, CA May 31, 2022 A new University of California, Irvine-led study identifies sex-specific circuits of muscle signaling to other tissues and that the organs and processes muscle impacts are markedly different between males and females. This new discovery provides insight into how muscle functions, such as exercise, promote healthy longevity, metabolism and improve cognition.

The study, titled, Genetic variation of putative myokine signaling is dominated by biologic sex and sex hormones, published in eLife, is the first to evaluate how genetic architecture influences muscle signaling to other tissues, highlighting that sex and estrogens are critical determinants of these processes.

Muscle is criticalfor maintaining metabolic state and disruption of musclefunction is a hallmark of diseases such as obesity, type 2 diabetes and cardiovascular disease, said senior author Marcus M. Seldin, PhD, assistant professor of biological chemistry at UCI School of Medicine.

Muscles secrete proteins called myokines, which play roles in a variety of processes by interacting with other tissues. Essentially, myokines allow skeletal muscles to communicate with organs such as the kidneys, the liver or the brain, which is essential for the body to keep its metabolic balance. Some of the process myokines are involved include inflammation, cancer, the changes brought about by exercise, and even cognition. Despite the clear relevance of myokines to so many physiological outcomes, the way these proteins are regulated and their effects are not well understood.

For this study, the research team performed a survey of genetic correlations focused on myokine gene regulation, muscle cell composition, cross-tissue signaling and interactions with genetic sex in humans. While expression levels of a majority of myokines and cell proportions within skeletal muscle showed little relative differences between males and females, nearly all significant cross-tissue enrichments operated in a sex-specific or hormone-dependent fashion; in particular, with estradiol. These sex- and hormone-specific effects were consistent across key metabolic tissues: liver, pancreas, hypothalamus, intestine, heart, visceral and subcutaneous adipose tissue. This study highlighted a few examples such as that muscle signals more to the pancreas in females, compared to males where liver is dominant.

We already know that skeletal muscle plays an integral role in coordinating physiologic homeostasis. In this study, we sought to understand how muscle interacts with metabolic tissues and illustrate the importance of considering the effects of genetic sex and sexual hormones when studying metabolism, said Seldin.

Moving forward, the research team plans to generate cell-based systems to evaluate some of the new hormones uncovered as part of this study and investigate why they signal differently between sexes.

City of Angels to help NJ veterans regardless of situation – New Jersey 101.5 FM

Since 2008, City Of Angels has been helping people deal with addiction and recovery throughout New Jersey. They were instrumental in helping Artie Lange go into rehab.

Many times their founder Kevin Meara will be in studio talking about addiction or co-hosting one of our games only to hit the phone during a commercial break to help an addict trying to get help.

One night he sent chills down our spine when he pulled 23 holy cards out off his pocket representing the funeral he attended that month.

Now City of Angels is expanding to help out veterans with the Fr Vincent R Capodanno Veterans Research and Wellness Initiative at City Of Angels. Meara explains:

You also do not want to disturb the wildlife you encounter, just keep your distance from the wildlife and continue hiking.

Bicyclists should yield to hikers and horses. Hikers should also yield to horses, but Im not sure how many horses you will encounter on the trails in New Jersey.

If you are thinking of bringing your dogon your hike, they should be leashed, and make sure to clean up all pet waste.

Lastly, be mindful of the weather, if the trail is too muddy, it's probably best to save your hike for another day.

The Coaching Industry Expands Beyond The Field Of Play Into Healthcare Education – Forbes

Health education and coaching continues to grow as a professional path.

Coaches are seeing more opportunities than ever, including educators and administrators who choose to expand their know-how into adjacent industries of healthcare, EdTech, human resources, and wellness organizations.

The higher education community recognizes the importance of a carryover model, demonstrated by Vanderbilts coaching program for the Center for Student Wellbeing inside their nursing school. Additionally, Harvard Medical School is exploring the need for certified health and wellness coaches in hospitals across the country.

Education and healthcare continue to strengthen their sometimes unintentional and coincidental relationship, often borrowing approaches to support better outcomes.

To gain a perspective on the approach, this reporter sat down with Integrative Nutrition Certified Health Coach, Kevin Ellis, a.k.a the Bone Coach, to learn more about how education has evolved coaching, as a profession, from the traditions of the athletic field to careers of study and impact.

Ellis was diagnosed with osteoporosis at the young age of 30, decades before its typical onset. Left with shrinking bone density and a lifetime of pharmaceutical dependence, he took ownership of his health and, in turn, developed a coaching network to address solutions more holistically.

Beginning Diagnosis

Rod Berger: Describe your diagnosis and your journey during the initial stages?

Kevin Ellis: It's pretty shocking when someone hears that a 30-year-old male has osteoporosis. Very weak porous bones are usually associated with someone later in life. Its a more common condition found in women 55 years and much older. Its very uncommon for a 30-year-old active male who's a former Marine to be diagnosed with the condition.

I had suffered from celiac disease for years, where gluten damages the villi in the intestine. I couldn't absorb nutrients and my body started raking those nutrients like calcium from my bones to serve other purposes. My body was tapping into its own reserves in my bones, causing osteoporosis in my early 30s.

All the doctors failed to recognize the connection and thanks to a physician's assistant recommending a bone density exam, it was discovered. After doctors painted a future filled with broken bones and medication, I realized I needed to figure some things out.

I started down this path of reading, researching and consulting with many different people. I needed a plan in place to be there for my wife and kids to have an active future. I made progress, improving my health and my bones.

Coaching Onset

Berger: When we think about healthcare, or our own health, there's so much education that goes into it. I wonder if we have the correct information to understand and comprehend our bodies and what proper functioning looks and feels like.

Ellis: Pharmaceutical intervention may address the symptom but might not address the underlying root cause. I took the holistic approach, addressing bone loss issues to get the right plan in place. I became an integrative nutrition coach and started coaching people one-on-one. I noticed similar issues with diet, digestion, absorption, lab testing, stress, sleep and exercise.

I then realized that this could be repeated for multiple people for greater benefit and turned it into a program that we call the Stronger Bone Solution. I brought in a team of experts, highly credentialed people that are some of the best in their fields. I mentioned how an audience of underserved people would benefit from building stronger bones and would they want to be a part of it. These experts were immediately on board. Many of them were in women's health and understood the need.

Berger: It says a lot that these individuals, already the best in their fields, found your mission and story so compelling to dive into the mix.

Ellis: The team is fantastic. Many are in the natural health field. Theres an educator at the Institute for Functional Medicine, a functional medicine practitioner, a doctor of physical therapy, whos an adviser to the National Osteoporosis Foundation (NOF), and the American Bone Health organization. We have a Harvard Ph.D. who focuses on stress and sleep, a dietitian, who's also a chef and a university professor, and then we have a whole team of coaches underneath.

Weve helped people in over 1,500 cities worldwide and thousands of people. Our business and coaching programs are on an upward trajectory. Many people understand there is more to it than just whats in a pill.

Connection to Marines and Father

Berger: Not everyone is driven to take what theyve learned and apply it to help others. Is there something in your DNA going back to when you chose to be in the Marine Corps thats always had that element of service inside you?

Ellis: My health journey started before I was even born. When my mother was five months pregnant, my father was told he had cancer and two months after I was born, he passed away. He was 35 years old. I had this fear that I was going to follow in his footsteps to an early grave and not have the opportunity to watch my kids grow up and experience the joys of being a dad.

My father was a Marine, survived combat in Vietnam, but ended up getting cancer from Agent Orange used in the war. I went into the Marines because I thought it would make my dad proud. But also do good in the world and help people. I was in the Marine Corps for five years and brought a very close personal connection to my father, a way to say goodbye to him. After Afghanistan, I decided it was time to move forward with my life and went to school to get an education. But unfortunately, that was when my health took a turn for the worse.

The Marine Corps provided a foundation for me to create a business that would reach, help and serve people.

Coaching Models and Mentoring

Berger: The coaching model is growing in many areas, from education and healthcare to executive coaching and personal coaching. Please talk about the why you feel a coaching model works well with traditional practice inside medicine?

Ellis: Coaches compress time and you will always want to work with someone who condenses the process. I saw an intervention gap in osteopenia and osteoporosis and I decided to utilize the skills I attained and apply them broadly.

I'm very oriented around systems, processes, and templates. The idea with our clients and coaching teams is to eliminate inefficiencies that they may experience in a conventional model where you go to your doctor, spend 15 minutes, book another appointment for eight months to a year out, without needed support along the way.

In our model, we lay out a plan for the individual. Then, every day of the week, we give support along the way with the experts who know how to help you best in a round-robin fashion. It changes the way people get information. When you can get people the correct information they need faster, it has the potential to achieve faster results.

Berger: It feels like going through this kind of coaching process would help develop a sense of agency. Is that something you have noticed?

Ellis: Good coaches don't create dependencies; they empower, educate and help you become your own best advocate. A doctor can cause dependency for some people. A coach helps with concurrencies. Its about reducing dependencies and increasing concurrencies to move a person forward on their best health path.

Most physicians are not people who came into their practice to just prescribe medication and not care about the patient. On the contrary, most want to help people. So we avoid creating a situation where this adversarial relationship of us vs. them mentality exists.

We work together to create a partnership in our healthcare. A doctor may have an education or training in a specific area and help with critical needs. But there are other things they're not going to help you with during a 15-minute consultation. We give our clients tools to share with their physicians to make bone health support easy, including tests to run. We add the science and when results come back, we give clients the guides and tools to sit down with their physician. It's a partnership with the physician.

Mentor a Million

Berger: How do you see serving underserved communities? Is there even more opportunity to dive in even deeper with this kind of support?

Ellis: I'm in the process of developing a program called Mentor a Million. I want to mentor a million kids and teach them how to be good, kind, contributing members of society and teach them good values. I want my business, Bone Coach, to create internship opportunities for these kids so they can work in a purpose-driven organization that's contributing to the betterment of society.

Its my long term vision and it's all going to come together. I can see it clear as day. In my eyes, I'm already mentoring a million kids, but we just haven't crossed paths yet.

According to the 2021 U.S. Health Coaching Market Report the health and wellness industry market has ballooned to $4.2 trillion supporting an exploding market of health coaches to the tune of over $7 billion.

The Annals of Family Medicine projects a shortfall of primary care physicians of nearly 50,000 providers by 2025. The industry is starting to explore health coaching not as a substitute, but as a vital response to expanding health care needs.

Schools and universities have been found to be feeder systems into this sector. Education, writ large, will most likely be looked at as the training grounds to support new interventions and career pathways.

Ellis might just be a reflection of a person and career path that current students explore in the future supporting industry personnel shortages and individual health needs across the country.

The One Doctor You Dont Have But Likely Need – TravelAwaits

Many retirees are concerned with staying physically and mentally healthy. As we age, our risk for diseases and injuries begins to increase. This is where integrative medicine can come into play.

As a primary care doctor and Osteopathic Physician (D.O.), I found looking for the root cause of illness and possible prevention much more challenging and rewarding than just diagnosing and medicating. Imagine if we can decrease breast cancer rates because of what we eat, how we reduce stress, and taking a look at advanced biomarkers and genetic testing. What if we can reverse diabetes and lower the risk of heart attacks and strokes and dementia? This is what gets me excited. Talking with my patients about alternative methods like using a sauna for 15-20 minutes four times a week is much more exciting than writing another diabetic script.

The term integrative medicine was born from combining the practice of so-called conventional medicine and complementary medicine. Conventional medicine is what most doctors practice. This is also called traditional Western medicine. Adding outside-the-box treatments such as chiropractic care, acupuncture, and other lifestyle recommendations like improving diet, supplements, herbs, exercise, stress management, and functional specialty labs results in the actual integration of the two disciplines. And we need both.

In some cases, especially those that are true emergencies, traditional medicine is lifesaving. But, in some cases, another prescription, procedure, or surgery is not going to help. One of the largest movements of integrative medicine in the United States is called Functional Medicine. Functional Medicine at its basic definition looks at the root cause of illness. While we look to undo the damage of the presenting complaints, doctors might integrate using traditional and complementary medicine to achieve healing. It makes sense. Look for why you have a problem and work backward. This contrasts the traditional medical diagnosis of a problem and writing a script to help.

You should look for where they received their integrative certificate. One of the most challenging programs is the Institute of Functional Medicine program. Its generally 3 additional years of study. Providers also have to take a board exam and present an actual patient case to be reviewed as part of the board examination. Once completed, they receive their certificate.

Another good idea is to read up on why someone is an integrative doctor. If they offer a free consultation, sign up and interview them. See if they have any reviews on their websites or if you can contact any of their current patients.

Pro Tip: Another consideration with an integrative doctor is how long they have been in practice. My suggestion is to find someone with 5 or more years of experience.

Integrative medicine is not covered because insurance companies cannot compensate properly for longer visit times or specialized and individualized medical and lifestyle care. This takes time. That is not how the traditional medical model was created. Insurance companies also dont recognize specialty blood work and other labs.

When you have a complex problem, a 10-minute office visit is not going to solve it. Integrative medicine takes time and is very complex. It is strategic in its approach. For example, if you have a medical condition called small intestinal bacterial overgrowth (SIBO) and have mold exposure, the questions become Which medical problem do you treat first? and How do you keep a patient from getting sick while treating?

Pro Tip: Have an HSA or FSA? Even though integrative medicine isnt covered by insurance, we take FSA and HSA payments. After every visit, a patient will receive an after-visit summary/receipt/invoice that they can turn into their insurance for potential partial reimbursement. Also, if prescribed by a doctor, we can fill out an insurance form allowing supplements to also be covered either with insurance or FSA/HSA funds.

There is no one answer to this question. But I can tell you why most of my patients see me. They are tired of being told everything is normal when they dont feel normal. They are looking for alternatives to hormone replacement therapy in the form of BHRT (bioidentical hormone replacement therapy).

My patients are also looking for someone who can help them become and stay healthy. Intuitively, you know that lifestyle medicine is important but you need someone to put it all together for you. You might be looking for the right diet for your body. You might want to lose weight because you know that even being 15 pounds overweight is causing many problems like diabetes, heart disease, and inflammation.

After an initial consultation with me, my patients receive health coaching, dietician consultation, and stress reduction tips. They learn how to sleep better and understand the importance of having a strong network of support. We also work through the right exercise programs.

I also order advanced medical lab testing for my patients. This is going way beyond and deeper than any traditional labs. Were doing stool studies, advanced mitochondrial studies, micronutrient testing, DNA aging testing, genetic testing, and more to improve lifestyle and function with an ultimate goal of disease prevention, longevity, and vitality.

Most of my patients come to me because they read something online and they get excited that there is a type of doctor out there who can help. Generally, they visit the Institution For Functional Medicine site to find a practitioner near them.

My patients come to me with a variety of issues: the guy who has eczema and is tired of using steroid creams learns gut health could be the root cause, the woman going through perimenopause or menopause who feels like she is going crazy but no one knows how to safely administer bioidentical hormones, the patient who has bloating after every meal and everyone tells her she is fine.

I also have patients who want to be able to put their own suitcases up on the top bin of an airplane well into their 70s and 80s. My patients value being functional. This is important. Not just growing old but growing old and staying functional. These are real patients with real issues who need more than conventional medicine to fix a problem.

I got into functional medicine because I knew there were solutions to all these problems. However, we were not taught how to fix them in medical school. Prescription medications can only do so much. I knew there were other ways to get to the root of the problems and not just mask them. My patients are living healthier, more fulfilling lives as we work toward the best solutions for each of them.

Hashimoto's Disease is an autoimmune disorder that affects the thyroid gland and "The number of people who have Hashimoto's disease in the United States is unknown. However, the disease is the most common cause of hypothyroidism, which affects about 5 in 100 Americans," according to the National Institute of Diabetes and Digestive and Kidney Disease. While it's unclear how many people struggle with Hashimoto's Disease, researchers do know it affects women more than men and Zoe Saldana can attest to that. The Avatar and Guardians of the Galaxy star revealed a few years ago she has the disorder, as do her mother and sister. To learn more about Hashimoto's Disease, Eat This, Not That! Health spoke with Dr. Michael Hirt, a Board Certified Nutrition from Harvard University and Board Certified in Internal Medicine and is with The Center for Integrative Medicine in Tarzana California who explained what to know. Dr. Hirt says, "Hashimoto's thyroiditis is a medical condition in which the immune system attacks your thyroid, a butterfly-shaped gland that sits just below your Adam's apple at the front of your neck. Early in the condition, patients may feel very little, but over time as the thyroid gland is destroyed by the immune attack, patients may begin to experience symptoms of insufficient thyroid hormone. These symptoms of low thyroid hormone levels typically include cold intolerance, fatigue, low mood, hair loss, constipation, generalized bloating, a weak voice, and weight gain."

Every cell in your body has a receptor for thyroid hormone. This means that healthy cells require healthy levels of thyroid hormone. Once the destructive process of Hashimoto's has compromised your gland's ability to pump out sufficient thyroid hormone, then patients will begin to experience the negative effects of low thyroid hormone blood levels. Much like the slow dimming of the lights at a movie theater just before the show starts, patients with Hashimoto's may not initially realize that their body is changing. Patients may think that they are tired because they didn't get enough sleep, that they are gaining weight because of their diet, or that they are bloated because of excessive salt consumption. Eventually, most patients will realize that something is very wrong and seek medical attention from their healthcare professional."

Dr. Hirt explains, "A simple blood test can determine the extent of the thyroid hormone deficiency and prescription thyroid hormone is available to replace the lack of hormone from the thyroid gland. Dr. Hirt lists the following signs to watch out for.

The Mayo Clinic states, "Hashimoto's disease is an autoimmune disorder. The immune system creates antibodies that attack thyroid cells as if they were bacteria, viruses or some other foreign body. The immune system wrongly enlists disease-fighting agents that damage cells and lead to cell death.

What causes the immune system to attack thyroid cells is not clear. The onset of disease may be related to:

According to the Mayo Clinic, "Most people with Hashimoto's disease take medication to treat hypothyroidism. If you have mild hypothyroidism, you may have no treatment but get regular TSH tests to monitor thyroid hormone levels.

T-4 hormone replacement therapy

Hypothyroidism associated with Hashimoto's disease is treated with a synthetic hormone called levothyroxine (Levoxyl, Synthroid, others). The synthetic hormone works like the T-4 hormone naturally produced by the thyroid.

Heres What Happens to Your Alcohol Tolerance as You Age – Well+Good

There are some folks who can seemingly drink and drink without feeling any consequences (well, at least in the short term), but as our college days grow further and further out of sight, that actually gets to be increasingly rare.

As we get older, our body's ability to process alcohol decreases due to a combination of changes," says Erika Schwartz, MD, an integrative medicine doctor who founded Evolved Science. "This is due to both natural aging, the buildup of exposure to toxins throughout life, as well as deterioration of enzymatic processes necessary to detoxify alcohol from our system."

Dr. Schwartz goes on to explain that as we age, muscle is replaced by fat and fat cells hold less water than muscle cells, largely diminishing the positive effect that muscles can have on the processing of alcohol. Not only does this mean your body cant metabolize alcohol as quickly as it did when you were younger, it also increases the direct damage to your liver from alcohol, she says.

That said everyone is different.

Some peopleincluding those who are elderlymay get wobbly after one alcoholic beverage, while others need a few drinks to feel any impact. Body size and composition are significant factors in alcohol detoxification and tolerance regardless of age, says Dr. Schwartz. Many [people who identify as] men tend to have larger bodies, which indicates more area to distribute alcohol in lowering the overall alcohol content. They also tend to have more muscle, which in turn leads to a higher water content, helping to dilute and diminish the effect of alcohol on the body. Also, some [people who identify as] women have less alcohol dehydrogenase, the enzyme that breaks down alcohol, leaving more alcohol in the bloodstream for longer periods of time. This is why the U.S. Dietary Guidelines recommend keeping drinking habits to a minimum, which they define as no more than two drinks per day for men and one per day for women.

Moderate to heavy drinking over an extended period of time may increase the risk of a dangerous type of irregular heartbeat in adults over 40 years of age, too cardiologist Suzanne Steinbaum, MD, DO, medical expert for American Heart Associations Go Red for Women movement previously told Well+Good. Atrial fibrillation, or AFib, occurs when the heart's upper chambers beat irregularly and can increase stroke risk fivefold if left untreated. The condition is estimated to affect 12.1 million people in the United States by 2030. Studies have linked higher alcohol consumption to increased risk of AFib. If you have been diagnosed with high blood pressure, your doctor may [also] advise you to reduce the amount of alcohol you drink.

Having progressively more and more trouble tolerating a glass of pinot is no fun, but there are a few things that can be done to mitigate the negative effects of alcohol. Dr. Schwartz suggests limiting sugar-laden mixed cocktails, which can increase the inflammatory effects of alcohol further and may cause you to drink in excess. Outside of that, studies on effects of alcohol in the body show that the kind of alcohol isn't as important as the amount over time. Again, she suggests limiting yourself to one, but preferably none (she is a doctor after all!).

Frequent physical activity may also help thwart the negative effects of alcohol. As we age, our bodies metabolize alcohol less efficiently and blood alcohol levels remain high in people who are less active and ill-prepared to detoxify its untoward side-effects, says Dr. Schwartz.

That said, rewinding to the part about cutting back on drinking, its worth noting that alcohol speeds up the aging process because regular drinking can result in a loss of skin elasticity, which may mean an increase in fine lines and wrinkles. Alcohol also decreases cellular efficiency throughout the entire body, making our vital organs function less efficiently, leading to chronic disease. Plus, its a depressant affecting behavior and making it difficult to think clearly and make executive functions, Dr. Schwartz explains.

And thats not all. While an occasional drink is fine, consuming more than what is considered a moderate amount of alcohol has been shown to have a significant effect on hormonal levels, particularly testosterone. Many studies of adults over the years have shown that consuming two to three drinks per week had a significant decrease in testosterone after three weeks, while others show alcohol intake may increase intestinal inflammation, says Brigid Titgemeier, RD.

Bottom line? Its always important to be mindful and honest with yourself about how many glasses you are consuming and how often. With that in mind, like most doctors, Dr. Schwartz would suggest working on cutting out alcohol versus working on getting your body to better tolerate it. Thinking about the price of getting sick can certainly help meet Dr. Schwartzs recommendation. If not, flavored sparkling water or a good ol mocktail can ease the transition.
