Introduction

Stem cell therapy is a form of regenerative medicine where progenitor cells - a biological cell that has a tendency to differentiate into a specific type of cell -are used to augment the body's innate ability to heal.Common sources of stems cells are from one's own body (autologous), as well as from fetal sources, such as umbilical cord and amniotic fluid.For orthopedic application, we are interested in mesenchymal stem cells for their innate ability to turn into cartilage, tendon, ligament, muscle and vasculature. The process includes harvesting these mesenchymal stem cells from an individual's bone marrow and adipose tissue, then delivering treatment specifically to the area of injury by way of image-guided injection.

Stem cell therapy is a single day procedure throughout which patients remain awake and comfortable. An anxiolytic is available as pre-medication to reduce anxiety by request, and a local numbing medicine is used throughout the procedure.

Taking care to not expose any stem cell tissues to agents that decrease viability,bone marrow is harvested from a patient's posterior pelvis in an area called the posterior superior iliac spine (PSIS). Using a special needle that is designed to enter the bone,marrow is withdrawn from several different directions. For an adipose stem cell harvest,tissues are normally harvested from the flank/abdominal region, using special needles designed to infiltrate non-toxic numbing medicine with saline and to gently aspirate adipose tissues.

The harvested stem cell tissues are then minimally processed to isolate the mesenchymal stem cells. Once isolated and using high-frequency ultrasound imaging,stem cells are guided into the target tissues.

In our in-house lab, we utilize flow cytometry to ensure optimal stem cell counts and viabilities, a bio-cabinet to process orthobiologics that ensures a sterile procedure, and tested protocols to produce the best stem cell yields.
